Home care after a hospital stay

Coming home from Bayhealth, ChristianaCare, Beebe, or TidalHealth? A caregiver helps with the bath, meals, and getting around. Call (302) 689-3240.

A caregiver serves a meal to an older adult in a living room.

Coming home from the hospital or rehab is a relief, but the first few weeks can be hard. Getting in and out of bed, showering, and keeping up with meals can suddenly feel like a lot. A Reliable Aid caregiver can be there to help while your loved one gets their strength back.

How we help after a hospital stay

  • Getting settled at home on the first day
  • Help in and out of bed, chairs, and the shower
  • Bathing, dressing, and grooming
  • Fresh meals and reminders to eat and drink
  • Medication reminders
  • Help keeping track of follow-up appointments
  • Laundry, fresh linens, and light housekeeping
  • Company, so they're not alone while they recover

Working alongside home health

If your loved one's doctor ordered nursing or therapy at home, a home health agency handles those visits. We take care of everything else in the day: the meals, the bathing, the company, and the hours in between. Many families use both. Home care vs. home health explains the difference.

Planning ahead

The best time to call is before discharge. Let us know the expected discharge date, where your loved one will be staying, and what the hospital team said they'll need. We'll plan the first visits around that.

Every caregiver is background-checked. We're insured and bonded, and we send a backup if your caregiver can't make a visit.

Paying for care after a hospital stay

If your loved one has Delaware Medicaid long-term care, personal care at home may be covered. We work with all three Delaware Medicaid health plans. See how Medicaid home care works.

Questions families ask

Does Medicare pay for help at home after a hospital stay?

Medicare can pay for skilled home health, like nursing or therapy, when a doctor orders it. It generally doesn't pay for help with bathing, meals, and daily tasks on its own. Here's what Medicare covers.

How long will we need help?

It varies. Some families need a few weeks of support, others need ongoing care. We can adjust the schedule as your loved one recovers.
